Job Description:
A company is looking for a Clinical Care Navigator.
Key Responsibilities
Provide clinical support and referrals through a clinical phone
queue
Assess risks and provide crisis support and safety planning for
high-risk members
Coordinate care and services among multi-disciplinary teams to
enhance member experience
Required Qualifications
Licensed as LCSW, LMFT, LPC, or psychologist
Minimum of 2+ years of direct clinical care experience
Expertise in crisis assessment and safety planning
Knowledge of crisis support and solution-focused clinical
interventions
Tech-savvy and able to efficiently manage multiple tasks
